The child must first pass the evaluation test to be administered by the Supervisor for the applied level.

Upon admission, the following must be submitted:

1. Accomplished Enrollment Reservation Form

2. 2 photocopies of birth certificate from the National Statistics Office (NSO) with the original copy only for verification purposes

3. 3 pcs 1x1 picture of the child on white background

4. 2 pcs 1x1 picture of each parent, guardian, “yaya”, or driver on white background for use on the fetcher’s ID card

5. Report card from previous level attended

6. For non-Filipinos: 2 photocopies of ACR for Chinese citizens/foreigners and the original copy of the Student Permit for non-permanent residents.

Upon enrollment, the parent/guardian is required to fill up a Medical Record form of the child. All pertinent data about the child’s health, especially any medical condition, allergies, medication and immunization must be indicated in the record.
